Job Summary:
We are seeking a dedicated and compassionate Home Health Occupational Therapist to provide professional therapeutic services to patients in their homes. The ideal candidate will assess patients’ physical and mental conditions, develop individualized treatment plans, and help patients improve their ability to perform daily living and work activities.
Duties and Responsibilities:
- Conduct comprehensive evaluations of patients’ conditions abilities, and functional limitations
- Develop and implement individualized treatment plans to meet patients' needs and goals
- Provide hands-on therapy and rehabilitation services in the home environment
- Educate patients and families on proper use of equipment and techniques to enhance progress
- Collaborate with interdisciplinary team members to ensure continuity of care
- Maintain accurate and complete patient documentation, including progress notes and treatment plans
- Stay current on advancements in occupational therapy through continuing education
- Other duties as assigned

- Master’s degree or higher in Occupational Therapy

- Employees work in an office environment sometimes with moderate noise levels, controlled temperature conditions and sometimes travel to patients’ homes where they may have direct exposure to hazardous substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures.
- Employees will have to travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Other homes may be cluttered, dirty, with an uncomfortable temperature.
