Job Summary
We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will be responsible for providing comprehensive case management services to patients in various settings, including outpatient and acute care environments. This role involves conducting diagnostic evaluations, managing medical records, and ensuring compliance with HIPAA regulations. The Case Worker will work closely with healthcare professionals in both Level I and Level II Trauma Centers to facilitate optimal patient care and resource utilization.
Duties
- Conduct thorough diagnostic evaluations to assess patient needs and develop individualized care plans.
- Collaborate with medical staff in the Pediatric Intensive Care Unit (PICU) to ensure continuity of care for patients.
- Manage and maintain accurate medical records while adhering to HIPAA guidelines.
- Implement utilization management strategies to optimize patient outcomes and resource allocation.
- Provide support and guidance to patients and their families throughout the treatment process.
- Coordinate outpatient services and follow-up care as necessary.
- Stay informed about current best practices in case management, physiology, and trauma care.
Requirements
- Bachelor’s degree in social work, psychology, or a related field; Master’s degree preferred.
- Experience in case management within acute care settings is highly desirable.
- Knowledge of diagnostic evaluation processes and utilization management principles.
- Familiarity with medical records systems and HIPAA regulations.
- Strong understanding of physiology as it relates to patient care.
- Previous experience working in a Level I or Level II Trauma Center is a plus.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team environment.
