What are the responsibilities and job description for the Caregiver I position at Choice Health At Home?
The Special Attendant-Caregiver is responsible for overseeing the quality care for patients and reporting pertinent information to the immediate supervisor in accordance with regulatory requirements. Complies with policies and procedures and current federal, state, and local standards, guidelines, and regulations that govern Provider agencies to assure each patient receives the necessary services to attain and maintain the highest possible functional status.
Essential Duties and Responsibilities:
- Daily Report
- Engage everyone with a smile
- Connect and identify new clients
- Drive to appointments
- Regularly performs tasks to assist client in meeting health care needs as directed by a Non Licensed Field Supervisor
- Regularly provides personal care and health related services of assigned caseload in order to maintain the client’s physical and emotional well being
- Consistently works cooperatively with other members of the health team to provide a safe and comfortable environment
- Lite cooking, cleaning and providing in-home assistance
- Laundry
- Stand-by for showers
- Take out trash
- Pick up prescriptions
- Escort clients to doctor/medical appointments, hair/nail and general appointments as well as to the movie theatre and grocery shopping
- Regularly bathes and dresses clients or assists them in bathing or dressing, combing and shampooing hair, cleaning dentures, oral hygiene and shaving
- Responsible for understanding, respecting, and following the health information privacy and security policies and procedures (HIPAA) and is personally accountable for same
- Provides emotional support to patient and family
- Consistently responds to requests in a friendly and cooperative manner
- Communicates changes in client condition to appropriate members of the care team
- Consistently projects a positive image when dealing with patients and their families
- Demonstrates sensitivity to patients needs at all times
- Lunch Breaks: May be required to be on site
- Other duties as assigned\
Requirements:
- Passion for people
- Previous experience caring for the elderly or disabled is preferred
- Must demonstrate competency in providing personal care tasks to the satisfaction of the supervisor of patient care
- Able to provide own reliable transportation to home visits
- Must be a minimum of 18 years of age
- Able to take call after regular business hours, including weekends and holidays If applicable
- Self motivated and able to perform with minimal supervision
- Current Driver’s License required
- NO Medical Field Certifications required
