We are looking for weekend LPN help.
Position Summary
The Hospice Licensed Vocational/Practical Nurse is responsible for delivering clinical care to a group of seriously ill patients served in their place of residence (home, SNF, ALF, etc.). The LPN will provide comfort for and help preserve the dignity of people at the end of their journey. .
As a Hospice LPN, you will:
Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Assure the continuity of care through delivery of quality patient care.
Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
Complete appropriate clinical and progress notes, documenting outcomes of interventions.
Assess patient/family response to care, ensuring communication of information to appropriate team members.
Requirements
Current LVN/LPN License
Valid Driver’s License
Valid Auto Insurance
CPR Certification
HomeCare HomeBase experience preferred
