Job Summary
The primary function of the PSC is to schedule patient services and coordinate with staff. The PSC is also
expected to cross-train as the Medical Records Specialist (MRS).
Job Qualifications
Education: High School graduate. Graduate of an accredited school of vocational nursing, preferred.
Licensure: If clinical, must have a current state license as a Licensed Vocational Nurse or Licensed Practical
Nurse. A multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) is
preferred. Current driver’s license. If required to make patient visits, a current CPR card must be required.
Experience: One year of experience in homecare (e.g., home health or hospice) is preferred.
Skills: Ability to establish and maintain effective working relationships with all segments of the staff. If an LVN,
nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation and valid and current auto liability insurance.
Environmental/WorkingConditions: Performs duties in an office environment during Agency operating hours.
Works in a routine office environment; noise level may be moderately high; must have the ability to work a
flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc. The ability to work a
flexible schedule, travel locally; some exposure to unpleasant weather.
1. Complete workflow tasks and other assignments to the PSC Responsible Position in the
Agency’s electronic medical record.
2. Prioritizes the scheduling of new patient admissions in coordination with the Admin or CM--
RN.
3. Participates in coordinating care with nurses, patients, and other interdisciplinary team members.
4. Provides effective communication to patients, staff members, other health care professionals
and referral sources.
5. Respond to Agency and patient needs professionally and creatively.
6. Demonstrates commitment, professional growth, and competency.
7. Participates in Agency-sponsored in-service training.
8. Participates in the Agency’s QAPI program, as directed.
9. If clinical, perform patient visits as assigned and complete documentation timely.
10. Promotes the Agency philosophy and administrative policies to ensure quality of care.
11. Cross-trains as the Medical Records Specialist (MRS) as needed.
