Per Diem Hospice Field MSW

*THIS IS AN ON THE ROAD POSITION (FIELD WORK), NOT REMOTE*

Comfort Choice Hospice Care is seeking a COMPASSIONATE Per Diem Medical Social Worker (MSW) to provide and assess the psychological, social, emotional, spiritual, and bereavement needs of hospice patients and their families. Our MSWs get to choose their own schedule and would cover cities within Los Angeles County!

Areas of Coverage Include:

• Culver City, West Los Angeles, Beverly Hills, Brentwood, Pacific Palisades, and surrounding

Per Diem MSW Responsibilities:

• Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
• Communicates findings to the registered nurse as well as the interdisciplinary group.
• Counsels patient and family/caregivers as needed in relationship to stress and other identified coping difficulties.
• Maintains clinical records on all patients referred to social work.
• Provides information and serves as liaison between patients and families/caregivers and community agencies.
• Assists physician and other team members in understanding significant social and emotional factors related to health problems and hospice issues.

Per Diem MSW Qualifications:

• A graduate of a bachelor's program in social work accredited by the Council on Social Work Education.
• Licensed with ACSW (If Bachelor's provided, not applicable to Master's)
• Minimum of one year's experience in hospice experience required.
• Understands hospice philosophy preferred.
• Demonstrates good verbal and written communication, and organizational skills.
• Possesses and maintains current CPR Certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Pay: $90.00 per visit
