What are the responsibilities and job description for the Community Health Worker position at Choice in Aging?
The Community Health Worker (CHW), reporting to the Lead Care Manager will work directly with the Enhanced Case Management (ECM) team to identify eligible individuals and ensure their safe transition from skilled nursing facilities to community living when possible. The CHW will be cross-trained in all case management, transitions, and diversions programs to cover if needed. The CHW works as a member of the Choice in Aging (CiA) team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.
Essential Duties:

- Work with older adults in Contra Costa County to help improve their quality of life by developing care plans, facilitating access to resources, advocating for client rights, and promoting self-sufficiency and empowerment.
- Locate, contact, and engage members who have been identified as candidates using a progressive approach.
- Document outreach and engagement attempts and modalities.
- Attend and participate in monthly management meetings.
- Participate in and/or direct functions related to fundraising and outreach.
Minimum Qualifications
1. High school diploma or equivalent required, experience in case management with older and/or disabled adults; nonprofit experience preferred.
2. Associate’s or Bachelor’s degree in related field preferred.
3. Working knowledge of MS office, Google suite, and basic internet usage.
4. Experience working with the Older Adult population preferred.
5. Bi-lingual English/Spanish or English/Tagalog desired.
