Responsibilities
- Conduct thorough patient assessments to develop personalized care plans
- Administer skilled nursing care in accordance with physician's orders and established protocols
- Monitor and document patient progress, ensuring accurate and timely charting
- Educate patients and their families on disease management, treatment plans, and self-care techniques
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate and ensure seamless care transitions
- Participate in case conferences and interdisciplinary team meetings to review patient progress and adjust care plans as needed
- Comply with all regulatory guidelines, including OASIS assessment and documentation
Qualifications
- Active Registered Nurse (RN) license in the state of Florida
- Minimum of 2 years of experience in home health care (preferred)
- Proficiency in OASIS documentation, required
- Ability to exercise initiative and independent judgment
- Strong clinical and assessment skills
- Excellent communication and interpersonal skills
- Proficient computer skills for accurate documentation and electronic medical records
- Reliable transportation and valid driver's license
