What are the responsibilities and job description for the Homeless Outreach Supervisor position at CHOICE of NY?
Purpose of the Role:
The Homeless Outreach Supervisor (HOW) is responsible for managing a team of Homeless Outreach Workers who provide wrap-around services to individuals living with a mental illness and/or experiencing homelessness. The Homeless Outreach Department is expected to engage individuals who are experiencing homelessness either on the streets or in shelters to provide advocacy, assist with connection to benefits, and housing. In the process, ensure adherence to policies, procedures, and regulations while safeguarding quality and driving positive and measurable outcomes.
Essential Functions of the Role:
· Supervise, train, and develop the (HOW) Homeless Outreach Workers in their ability to address their client’s immediate and emerging needs, resolve issues, advocate, and connect clients to needed resources such that their lives become stable.
· Lead and coach assigned team in a manner that they become increasingly independent in their capability to manage day-to-day interactions by developing their problem solving and advocacy skills.
· Review HMIS data and notes accurately, thoroughly and timely, verifying documented activities and outcomes into case management systems.
· Eliminate potential issues related to the Grant Funding requirements, align documentation and remain “audit ready”.
· Conduct supervision meetings with each HOW monthly, reviewing key metrics.
· Proactively communicate with Director on any emerging issues and needed adjustments to the Department.
· Connect with each assigned client minimally 4 x per month.
· Review assigned caseload with Homeless Outreach Worker 2 times per month to ensure effective case management (as noted above); adjust as needed.
· Develop and maintain productive and results oriented relationships with mission critical persons at outside organizations.
· Attend meetings, as necessary, at the Department of Community Mental Health, to review client cases.
· Complete street outreach assessment (VI-SPDAT) as requested by HOW Supervisor.
· Using the full capacity of one’s ongoing experience and training, demonstrate progressive ability to problem solve, advocate, mediate and handle increasingly complex tasks related to care coordination.
· Do not settle for “because that’s the way it has always been done”, rather be fearless in the pursuit of excellence and achieving the needed outcomes for our clients and the agency at large.
· Other activities as assigned.
For all field work:
· Track one-on-one with clients needing services, screen to determine eligibility.
· Monitor 3-5 intakes per shelter visit and total 10-15 intakes per week per (HOW).
· Proactively report to the Director all trends, relationships, and developments in the field.
· Track activities and outcomes monthly via excel spreadsheet.
Decision Making Authority:
Decisions regarding client care, case management, and outreach are to be made with the guidance and collaboration of the Director. It is expected that the Homeless Outreach Supervisor will show an increasing ability to make decisions independently on routine matters as knowledge and experience progresses.
Working Relationships:
Internal: Homeless Outreach Workers, Client Care Supervisor, Director, Client Care Coordinators, and other members of the clinical team.
External: Dept. of Social Services, Shelter Programs, Mental Health Providers, Primary Care Physicians, DCMH, Probation Officers, Police Officers and other local officials, Legal Services, District Attorney’s Office, etc.
Work Schedule: Monday – Friday, 9am – 5pm with some flexibility around client and outreach needs.
Physical Environment:
o Traditional office environment.
o Shelters/local streets
o Must be comfortable attending appointments at various agencies, facilities and client’s homes.
Physical Demands: Must be able to drive a motor vehicle, stand for long periods of time, and carry up to 20 lbs.
Qualifications for this Role:
· Bachelor’s Degree required.
· Minimum 2-3 years of care related experience in the areas of advocacy, mediation, and crisis management.
· Valid Driver License required and a driving record that will permit the use of an Agency vehicle to transport clients to appointments or other activities as needed.
· Computer literacy required.
· Proven ability to empathize with the clients we serve.
· Tenacity and passion for this work with the ability to balance objectivity with empathy.
· Must be able to comfortably interact with persons having mental health issues, periods of instability (ex. Lack of shelter, food, clothing, or support), recently released from an institution (ex. Hospital or prison), limited comprehension and/or ability to manage through complex scenarios.
· Bilingual English/Spanish a plus.
