What are the responsibilities and job description for the Companion/Caregiver position at Choices of Change, LLC?
We are looking a professional, dependable, motivated person who is passionate about caring for individuals with developmental disabilities & behavioral issues.
This position would be perfect for someone wanting daytime hours, with some flexibility.
Responsibilities can include but are not limited to:
Assists clients in all activities of daily living and goals set in their personal plan.
Transport individuals on community outings per service requirements, as well as appointments, errands, etc.
Perform light housekeeping chores as well as prepare nutritious meals.
Accurately complete daily documentation and other record keeping duties online and carry out all assignments as required.
Willing and able to competently perform all duties, and follow instructions, policies and procedures set by Choices of Change and all regulating agencies.
Willing to travel to Dublin for trainings.
Minimum Requirements:
· Must have reliable transportation and can provide valid proof of car insurance for said automobile & a good 7 year motor vehicle report
· Must have a current valid Driver’s License & Social Security card
· Must pass local and national background criminal background check
· Must have TB test results & physical (less than a year old)
· Must have or be willing to get CPR/First Aid Certified within 60 days
· Must have access to internet daily

Pay: From $12.00 per hour
Expected hours: No more than 40 per week
