What You Will Do
As a Medical Social Worker you will be a valued member of an interdisciplinary team whose primary function is to provide diagnostic evaluations of the psychosocial problems of the patient/family unit, defining social service goals for alleviating these problems, and help to provide both the counseling and casework needed to meet these goals.
As part of a dedicated team of caregivers, the Medical Social Worker is part of delivering services addressing the physical, emotional, and spiritual needs of patients of all ages and their families. Make a difference in serving patients and families on a one-on-one basis!
Job Responsibilities
- The Medical Social Worker (LSW) performs the initial psychosocial, emotional, spiritual, and bereavement assessments in the development and implementation of a goal-directed, interdisciplinary, care plan.
- Conducts ongoing assessments of patient/family needs and counseling as required
- Provides short-term crisis intervention and individual or family counseling when indicated
- Delivers services under the direction of a physician who approves the plan of care
- The Medical Social Worker (LSW) observes, assesses, and brings information to ID team meetings regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family
- Assumes the active role of advocate for the patient/family unit
- Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and facilitate related referrals
- Documents comprehensive psychosocial, emotional, and spiritual assessments clearly and concisely in a timely manner
- Provides education to the patient and family related to issues of death, dying, and bereavement as needed
Qualifications
- Master’s degree in Social Work preferred
- Bachelor's degree considered
- One year of Licensed Social Worker (LSW) experience in a healthcare setting
- Currently licensed in the state of Ohio
- Ability to work independently, make accurate, and at times, quick judgments.
Pay: $29.48 - $32.16 per hour
