What are the responsibilities and job description for the Hospice Chaplain position at CHP Home Care & Hospice?
CHP Home Care & Hospice is in need of a hospice chaplain or pastor with a gift or calling for end-of-life care support. This is a part-time, paid position with mileage reimbursement included. Provide in-home visits to patients and families in our service territories of Archbold, Bryan, Defiance, and Paulding. The hours are PRN (as needed), and will serve as a back-up to the main chaplain.
RESPONSIBILITIES
You will assist in the development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care. Provide spiritual support in accordance with the ongoing wishes and needs of the patient/family in accordance with their religious preferences or definition of spirituality. Work in close collaboration with local clergy when desired by the patient. Maintain and submit written clinical records including an initial evaluation, the care plan, and visitation notes. Evaluate the patient's and family's responses to, and the effectiveness of, spiritual counseling. Work with the Nursing Supervisor and clinical staff on the scheduling of visits. Participate in bi-weekly interdisciplinary team (IDT) meetings, working with other professionals in resolving spiritual/ethical issues.
REQUIREMENTS
Applicant must be a graduate of an accredited seminary or school of theology, have appropriate certification in a hospital or pastoral ministry, or have experience with hospice or grief-related programs. The chaplain must be open, flexible, and ecumenical. Experience in working with patients/families dealing with serious illness and death is also a plus.
This is a paid, part-time/as-needed position.
