JOB TITLE: Health Educator
EMPLOYMENT TYPE: Part-time; 5 hours/week- TEMP approx. 8-9 months.
SCHEDULED HOURS: To Be Determined
PROGRAM/LOCATION: Micro Grants, Manchester, CT
ABOUT THE POSITION:
In collaboration with the Norma and Natale Sestero Fund, CHR’s Health Educator will provide improved health outcomes to individuals in Glastonbury Connecticut with serious and persistent mental illness and substance use diagnoses through nutrition and exercise counseling and smoking cessation support. The Health Educator will work with individuals to evaluate health needs, establish health and wellness goals related to fitness, nutrition and overall health, and support these individuals’ efforts to meet those goals.
By:
- Working with adult and/or child clients as assigned.
- Providing individuals with serious and persistent mental illness and chronic health conditions assistance with achieving and maintaining their highest level of health.
- Linking individuals with appropriate treatment sources.
- Coordinating care with medical and collateral providers.
- Assisting individuals with accessing health care services.
- Providing health and wellness education and training, in individual and group formats.
- Assisting individuals with setting and meeting wellness and recovery goals.
- Maintaining records of services provided as well as indicators of health (i.e. BMI, weight, smoking cessation, blood pressure, etc.).
- Tracking progress on health and wellness goals and make recommendations for next steps.
DUTIES & RESPONSIBILITIES:
- CHR Health Educator working with adult clients provide services including:
- Care Coordination
- Health Promotion
- Health Education
- Patient and Family Support
- Linking to Community Support Services
- Collaborates with other CHR team members to provide improved medical and behavioral health care coordination and outcomes for clients.
- Works to reduce health care disparities whenever possible.
- Demonstrates competency in age related or specialty issues and developmental needs for each population served.
- Assists in obtaining access to medical and wellness services including tobacco cessation intervention services, medical services, fitness services.
- May provide client transportation as needed.
- Provides wellness and health-promoting lifestyle interventions such as, but not limited to nutritional counseling, smoking prevention and cessation, obesity reduction, increasing physical activity.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision, trainings and other meetings as assigned.
- Additional duties as required.
QUALIFICATIONS:
Education: BA/BS in human services, medical, or related field required.
Experience: Previous experience as a health educator preferred. Experience in behavioral health setting preferred. Experience working in integrated care setting preferred.
Licensure/Certification/Registration: Valid driver’s license. Certification as a Health Education Specialist (CHES) preferred. Other Health Education certifications will be considered. CPR/First Aid Certification within 6 months of hire.

COMPENSATION*:
$23.5681/hour min, and up commensurate upon experience.
*Actual rates are determined at the time of offer and are based off relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
