Position Summary :

Position Overview : We are seeking a compassionate, dedicated, and experienced Case Manager to join our team. This role involves regular face-to-face interaction with children in care, most of whom have suffered severe abuse or neglect. You will be responsible for providing concrete services, coordinating with service providers, and engaging with family members / guardians where appropriate. You will also manage documentation and ensure all client needs are met in a timely and organized manner.

Key Responsibilities :

  • Identify and access resources for children in care and connect them with appropriate community services.
  • Collaborate with clinical therapists to update treatment plans and provide recommendations.
  • Attend and contribute to supervisory meetings, staff meetings, and clinical consultations.
  • Maintain ongoing contact with clients to track their progress toward treatment goals.
  • Review and develop treatment plans in coordination with the client, their birth family, and DHHS workers.
  • Maintain accurate, up-to-date client files and documentation in compliance with licensing standards.
  • Engage with families and provide case management services, including linking them to necessary resources.
  • Attend court hearings, complete court reports, and ensure timely submission of documentation.
  • Supervise family visits and complete home visits as needed, ensuring compliance with contract requirements.
  • Maintain flexible work hours as needed to meet the needs of clients and families.
  • Provide on-call support for emergencies and ensure all mandated reporting (e.g., 3200's) is completed as necessary.

Qualifications :

  • Bachelor's degree in human services or related field, or Master's degree in relevant field (BSW or MSW preferred).
  • 2 years of experience in a Child Caring Institution, child placing agency, or mental health field.
  • LLBSW, LBSW, LLMSW, or LMSW preferred.
  • Valid Michigan Driver's License.
  • Experience working with children with behavioral, emotional, or mental disorders, preferably in a residential treatment setting.
  • Proficient in computer applications and comfortable with documentation and case management tools.
  • Ability to work collaboratively with team members, families, and external service providers.
  • Strong communication skills, both written and verbal.
  • Knowledge of trauma-informed care and sensitivity to cultural differences.
  • Ability to manage a flexible schedule to meet client and family needs.
  • Additional Skills and Attributes :
