General Summary:
Under the direct supervision of the Behavioral Health Services Manager, the Behavioral Health Consultant (BHC) is responsible for patient care and a variety of clinical tasks using the Primary Care Behavioral Health (PCBH) model. By using the Primary Care Behavioral Health (PCBH) model, the BHC collaborates with primary health care providers and integrates best practices related to behavioral interventions and strategies. The BHC will provide skill training through psycho-education, patient education strategies and develop a behavioral treatment plan for patients.
Major Areas of Responsibility:
- Provides counseling, therapy, education and treatment plans to patients in brief consultative sessions based on PCBH model;
- Assist and consult with primary health care providers in assessing behavioral health concerns, psychosocial problems, evaluation of care plans, and referrals;
- Strategize and coordinate care with patients, families, and providers to address behavioral health issues and maximize wellness;
- Assist in coordination of care with outside agencies as needed;
- Balance clinical work, timely documentation in EMR, and other administrative responsibilities;

- Knowledge of the Primary Care Behavioral Health (PCBH) model
- Knowledge of clinical pathways and protocols in primary care
- Exceptional interpersonal communication skills that allow for regular and effective communication with a variety of people, including patients, visitors, providers, and senior management;
- Able to “think on your feet” and collaborate/communicate effectively with the goal of fostering an environment of teamwork;
- Strong overall computer skills with experience working within an EHR;
- Able to organize, direct, prioritize, and delegate work appropriately.
Education and Experience:
- Current Georgia Licensure as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Licensed Clinical Psychologist
- Degree in Counseling, Marriage and Family Therapy, Social Work, or Clinical Psychology
