POSITION SUMMARY :
The Annual Wellness Certified Medical Assistant (ACMA) works with the Primary Care Provider Team and the Outreach and Engagement Team to facilitate a comprehensive Annual Wellness visit. The Annual Wellness CMA is expected to review and complete, documentation related to previsit planning and care delivery for Annual Well visits (AWV) and Gap in Care Closure. Through these interventions, the ACMA will ensure optimum patient-to-doctor flow.
KEY RESPONSIBILITIES :
- Scheduling patients for Annual Wellness.
- Reviews and prepares medical records to ensure completeness prior to patient appointment and obtains reports / results that are not included in the chart. Documents visit, observations, and related activities.
- Assists with gathering data for the patient history and physical assessment.
- Checks patient vital signs and body measurements when Bluetooth capability is activated and available.
- Documents patient stated information : vital signs, pain assessment, height, and weight in the patient chart.
- Explain Annual Wellness visit instructions to the patient, as appropriate. Provides teaching to patients and families within the scope of the knowledge base.
- Participates in an interdisciplinary workgroup(s) with Providers, Staff, IT, and outreach staff to co-develop system-wide standards and guidance
- Identify growth opportunities and performance improvement potential
- Completes clinical tasks are needed to prepare patients for Annual Well Visits (AWV), before appointments.
- Communicate important information to key stakeholders proactively to keep providers abreast of telehealth findings.
- Track progress and outreach to patients as needed.
- Manage and organize Medicare portals to ensure gaps are complete and data is entered into the system.
- Ability to work at multiple Christ Community Health Center locations.
Qualifications / Required Skill :
Education : High School Diploma or Equivalent. Associate degree preferred.
Experience : Minimum Three (3) years’ experience in a Clinical area required.
Licenses or Certifications : Active Certified Medical Assistant required
CPR certified
