PAY RATE: $15.41 Hourly (Pay is negotiable based on experience)
*This position will be located in Rockcastle County with occasional travel to Jackson and McCreary counties.

Provides partnerships with families in need, providing support and direct services while connecting them with resources in their community.
Essential Job Functions:
- Provides case management services; home visits; networking with agencies in assigned areas. Creates individualized action plans.
- Responsible for the implementation of seasonal initiatives such as Christmas Distribution and components of School Readiness Program.
- Responsible for keeping required records and providing required reports for the program.

Qualifications
Education:
High School Diploma or Equivalent In General Studies Required
18 Months Additional Training In related field Preferred
Years of Experience:
3 or more years of successful experience in related field or combination of education and experience.
Certifications and Licensure:
Possess a Valid Kentucky Driver's License and Maintain a Safe Driving Record Required
CAP Driving Certification* (Ability to Obtain within first 90 days) Required
*In order to drive personal vehicle on CAP business, must obtain and maintain a policy of motor vehicle insurance coverage on privately-owned vehicle with insurance coverage amounts of $50,000 bodily injury per person per accident; $100,000 bodily injury for all bodily injuries per accident; and $50,000 property damage per accident.

Ability to reach, push, pull, bend, stoop, stand, walk
Lift 50 lb from floor to waist
Lift 50 lb from waist to shoulder
Lift 25 lb from shoulder to overhead
Carry 50 lb for 25 ft
Lifts/moves items with unknown weight or that are awkward shapes and sizes with assistance
Have good visual acuity with corrective lenses
Have good hearing capabilities
