Job Description
We are currently seeking a PRN Registered Nurse/Case Manager to join our dedicated team. This position will cover Senior Living Communities from Central Phoenix to the East Valley, helping ensure our residents receive the highest level of personalized care in a supportive, compassionate environment.
As a Registered Nurse/Case Manager, you will be responsible for assessing, coordinating, and managing the overall care of patients in their homes or senior living communities. Care is provided 90% of the time on senior living campuses. You will work closely with the interdisciplinary team to develop and implement care plans, ensuring the needs of each patient are met. This is an as-needed role that offers flexibility while making a significant impact on the lives of seniors.

Responsibilities
- Case Management: Develop and implement individualized care plans for patients, coordinating with physicians, family members, and other healthcare providers to ensure optimal care.
- Patient Assessment: Conduct thorough assessments of patients’ physical, emotional, and social needs and update care plans as required.
- Care Coordination: Ensure the timely delivery of care services, including scheduling visits and coordinating with other healthcare professionals.
- Medication Management: Oversee medication administration, monitor for side effects, and educate patients and families on proper medication use.
- Patient Education: Provide education on health management, disease prevention, and improving quality of life to patients an their families.
- Documentation: Maintain accurate and up-to-date records for all patient visits, including care plans, progress notes, and communication with the care team.
- Advocacy: Act as an advocate for the patient’s needs, ensuring that care is delivered in accordance with the patient’s preferences and goals.
- Collaboration: Work closely with physicians, therapists, and other care team members to ensure a coordinated approach to patient care.
- Active Registered Nurse (RN) license in Arizona with current, valid credentials.
- Must pass background check and meet all health and safety requirements.
- Valid driver’s license and reliable transportation.
- Two years’ Registered Nursing experience in Home Health required.
- Strong assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently, manage time effectively, and provide compassionate care to seniors.
- Ability to travel to Senior Living Communities across Central Phoenix to the East Valley.
