Job Summary
The Client Advocate Specialist provides comprehensive case management services to children, adolescents, and their families. This role involves working closely with clients to identify and address their needs, develop personalized plans for improvement, and facilitate access to relevant resources and support.
Key Responsibilities:
Provide case management services to clients, including assessments, goal-setting, and plan implementation.
Develop and maintain relationships with community agencies, organizations, and healthcare providers to ensure seamless service delivery.
Conduct regular home visits, phone calls, and in-person meetings with clients to monitor progress, provide support, and adjust plans as needed.
Collaborate with multidisciplinary teams to ensure comprehensive care and coordination of services.
Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
Requirements
Bachelor's degree in Social Work, Psychology, or a related field preferred.
Three years of experience in mental health or a related field strongly preferred.
Valid Ohio Driver's License and driving record that allows employee to be covered on agency policy.
Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
