What are the responsibilities and job description for the Bi-lingual Case Manager position at Christian Community Action (CCA)?
Position Summary:
We are seeking a compassionate and dedicated Case Manager to coordinate activities within our Rescue and Restorative Programs and provide comprehensive case management services to clients. This role is crucial in supporting individuals on their journey toward stability, self-worth, and self-sufficiency by guiding them through educational, financial, and career-focused initiatives. Programs may include, but are not limited to, ESL, Computer Literacy, Financial Literacy, GED preparation, and Job Search Training.
To be successful in this role, the Case Manager must stay informed on educational and training trends, cultivate relationships within the community, and foster a supportive environment that empowers clients to achieve their goals.
Key Responsibilities:
With limited supervision and independent decision-making, the Case Manager will:
- Provide case management with strong time management and organizational skills.
- Conduct assessments and needs analysis to develop individualized action plans for clients.
- Build and maintain relationships with clients and community partners to strengthen and expand CCA’s Restorative Program.
- Regularly monitor client progress through scheduled appointments and update case plans accordingly.
- Maintain detailed case notes and documentation in the database to track client progress.
- Offer prayer support upon request and provide spiritual guidance as appropriate.
- Oversee and coordinate all initiatives within CCA’s Restorative Programs, ensuring effective execution of Job Skills Training, GED, ESL, Computer, and Financial Literacy programs.
- Research and develop curriculum and program strategies tailored to client needs.
- Assist clients with securing income, education/training, and necessary support services to enhance their self-sufficiency.
- Engage and mentor program participants, ensuring they remain motivated and actively participate in job readiness programs.
- Establish and foster partnerships with school districts, volunteers, and other community organizations to enhance program sustainability.
- Serve as a liaison between program participants and partner agencies, improving communication and service delivery.
- Utilize evidence-based practices, such as motivational interviewing, trauma-informed care, and harm reduction strategies, to effectively serve clients.
- Provide direct spiritual support and mentoring to individuals and families in alignment with their personal faith journey.
- Perform additional duties as needed to support the mission and goals of CCA.
Qualifications & Skills:
- Proven ability to manage multiple priorities and meet deadlines in a fast-paced environment.
- Strong written and verbal communication skills, with the ability to engage effectively in presentations and interpersonal interactions.
- Spanish fluency is required.
- Demonstrated ability to build relationships and collaborate with diverse groups.
- Bachelor’s degree in Social Work, Behavioral Science, or a related field from an accredited institution, plus three years of direct case management experience OR an equivalent combination of education and experience.
- Experience working with individuals in crisis and applying social work principles to support vulnerable populations.
- Active membership in a local church is required.
- Proficiency in Microsoft Office Suite (Excel, Word, Teams) and case management software.
- Strong ability to network with community leaders and establish partnerships to expand program reach.
- Experience leading and managing teams of volunteers.
- Ability to engage with diverse faith traditions to provide meaningful support to clients and families.
Work Environment & Physical Demands:
