We are seeking a compassionate and organized Registered Nurse/LPN to join our team at Christian Community Homes. As a Long-term Care Coordinator, you will play a key role in coordinating the care of our residents and working closely with our interdisciplinary team to achieve optimal outcomes.
Main Responsibilities
- Coordinate the care of residents, including developing and implementing care plans.
- Collaborate with the healthcare team to ensure seamless transitions and continuity of care.
- Monitor and report changes in resident condition and status to the Director of Nursing and other relevant staff members.
- Maintain accurate and comprehensive records of resident care and progress.
Requirements
- Degree from an accredited nursing program for RN/LPN.
- Active licensure as a registered nurse (RN) or licensed practical nurse (LPN) in the state of Wisconsin.
- Minimum 1 year of experience in long-term care or geriatric nursing.
- BLS certification required.
