Job Description:
- Essential Duties:
- Initiate resident-centered care plans by evaluating the unique needs of each individual and collaborating with our care planning team to develop personalized care strategies.
- Listens to concerns from residents, families, and physicians, addressing them promptly and professionally.
- Recognize signs and symptoms, using initiative and proper judgment to coordinate interventions and ensure seamless care delivery.
- Foster open communication among nursing staff, physicians, pharmacists, residents, and family members to maintain accurate records and promote cohesive care.
- Maintain confidentiality and respect residents' rights, adhering to strict privacy guidelines.
- Ensure precise documentation through electronic medical record platforms, guaranteeing accuracy and accessibility.
- Prepare, administer, and document medications according to physician orders, maintaining an up-to-date inventory and disposing of expired medication per protocol.
- Notify relevant parties about resident concerns, incidents, and progress through clear and timely communication.
- Detailed assessment findings, interventions, and follow-up actions are meticulously documented to facilitate informed decision-making.
- Supervise, coach, and discipline nursing support staff, escalating any concerns to the Director of Nursing or designee.
Basic Qualifications & Experience:
- A Registered Nurse (RN) license in good standing within Colorado is mandatory.
- 1-2 years of experience in an assisted living or skilled nursing setting, preferably with a focus on dementia care.
- Advanced education or certification in areas such as Eden Associates, Alzheimer's and dementia certifications, wound care programs, or management education can enhance your candidacy.
- Prior experience in a supervisory role and familiarity with electronic medical record platforms are highly valued.
- Superior customer service, communication, computer word processing, email management, and database skills are essential.
