\rResponsibilities:

- Gather and assess resident data to create personalized care plans \r
- Work closely with the care team to ensure each resident receives the support they need \r
- Stay up-to-date with MDS regulations and best practices \r
- Train and support staff to ensure understanding of MDS processes \r
- Participate in quality assurance efforts to continuously improve \r
- Support residents and families through clear and professional communication \r
- Collaborate with the billing team to ensure assessments align with reimbursement requirements \r
- Mentor new team members and guide them through the process \r
Requirements:

- Bachelor's Degree in Nursing (BSN) preferred \r
- Minimum 2-3 years of experience as an RN in long-term care or skilled nursing community \r
- Proven experience with MDS assessments and care planning \r
- In-depth knowledge of Medicare and Medicaid regulations \r
- Proficiency in electronic health record systems and MDS software \r
- Thorough understanding of the Resident Assessment Instrument (RAI) process \r
- Ability to work collaboratively in a fast-paced healthcare environment \r
