MDS Coordinator RN

Job Title: MDS Coordinator (On-Site)

We are seeking a detail-oriented and organized Registered Nurse (RN) to join our skilled nursing community in Colorado. As an MDS Coordinator, you will play a key role in assessing residents, developing care plans, and ensuring everything stays on track with Medicare and Medicaid guidelines.

Duties and Responsibilities:

• Gather and assess resident data to create personalized care plans.
• Collaborate with the care team to ensure each resident receives necessary support.
• Complete and submit MDS assessments on time to maintain smooth operations.
• Stay up-to-date with MDS regulations and best practices.
• Train and support staff on MDS processes.
• Participate in quality assurance efforts to identify areas for improvement.
• Communicate effectively with residents and families.
• Work with the billing team to align assessments with reimbursement requirements.
• Mentor new team members and guide them through the process.

Qualifications:

• Current RN license in Colorado
• Bachelor's Degree in Nursing (BSN) preferred
• MDS Certification preferred
• 2-3 years of experience as an RN in long-term care or skilled nursing
• Experience with MDS assessments and care planning
• In-depth knowledge of Medicare and Medicaid regulations
• Clinical assessment skills and attention to detail
• Proficiency in electronic health record systems and MDS software
• Excellent communication and interpersonal skills
• Ability to manage time effectively and prioritize tasks
• Understanding of the Resident Assessment Instrument (RAI) process
• Ability to work collaboratively in a fast-paced healthcare environment
