Christian Living Communities is seeking a skilled MDS Coordinator (RN) to join our team. As an MDS Coordinator, you will be responsible for assessing residents, developing care plans, and ensuring compliance with Medicare and Medicaid guidelines.

Key Responsibilities

• Assess residents to inform care planning decisions.
• Develop personalized care plans that meet the unique needs of each resident.
• Ensure timely completion and submission of MDS assessments.
• Stay up-to-date with MDS regulations and best practices to maintain high-quality care.
• Train and support staff to understand MDS processes.
• Contribute to quality assurance efforts by identifying opportunities for improvement.
• Maintain clear and professional communication with residents and families.
• Work with the billing team to align assessments with reimbursement requirements.
• Mentor new team members and guide them through the process.

Requirements

• Current Registered Nurse (RN) license in the state of Colorado.
• Bachelor's Degree in Nursing (BSN) preferred.
• MDS Certification preferred.
• Minimum 2-3 years of experience as an RN in long-term care or skilled nursing community.
• Proven experience with MDS assessments and care planning.
• In-depth knowledge of Medicare and Medicaid regulations.
• Strong clinical assessment skills and attention to detail.
• Proficiency in electronic health record systems and MDS software.
• Excellent communication and interpersonal skills.
• Demonstrated ability to manage time effectively and prioritize tasks.
• Thorough understanding of the Resident Assessment Instrument (RAI) process.
• Ability to work collaboratively in a fast-paced healthcare environment.
