This position involves working collaboratively with our care planning team to develop and implement personalized care plans for our residents. You will be responsible for assessing resident needs, communicating effectively with stakeholders, and documenting all interactions and care activities.

- Assess resident needs and develop comprehensive care plans in collaboration with our care planning team.
- Communicate effectively with residents, families, and healthcare professionals to address concerns and promote seamless care delivery.
- Monitor residents' health status closely, identifying potential issues and implementing interventions accordingly.
- Maintain accurate and confidential records, adhering to strict guidelines and protocols.
- Document assessments, interventions, and follow-ups to inform care decisions and drive continuous improvement.
- Collaborate with nursing support staff to ensure they possess the necessary skills and knowledge to deliver exceptional care.
Qualifications:
- A valid Registered Nurse (RN) license in the State of Colorado.
- 1-2 years of experience in an assisted living or skilled nursing setting.
- Advanced education or certification in relevant areas, such as Eden Associates, Alzheimer's and dementia certifications, or wound care programs.
- Prior experience in a supervisory capacity and proficiency in electronic medical record platforms.
- Excellent customer service, communication, computer, and database skills.
