What are the responsibilities and job description for the Family Medicine Center, NP/PA position at Christiana Care Health System?
The ChristianaCare Family Medicine Center is looking for an experienced APC to join our diverse team of Physicians, APCs, and Resident Physicians. This is an academic practice with two Wilmington locations providing primary care to adults and children. The locations are 1401 Foulk Road Wilmington DE 19803 (Foulk Road Family Medicine Center) and 501 West 14th Street Wilmington DE 19808 (Wilmington Hospital Family Medicine Center).
The selected candidate will rotate through both sites with both in-person and virtual sessions; providing chronic disease management, transitions of care, and acute access to our complex patient population. Virtual sessions will be at least 1 day per week and can work remotely once through orientation. The providers at this practice are proud to also offer routine wellness visits, women’s health services, LGBTQ comprehensive care, substance use disorder treatment, and a multitude of procedures.
