Job Summary:
We are seeking a compassionate and skilled Home Health Assistant to join our team at ChristianaCare. As a key member of our Community Health Service team, you will work under the supervision of a registered nurse and/or therapist to provide high-quality patient care in the comfort of their own homes.
About the Role:
- You will be responsible for providing personal care services to clients as established in their Plan of Care, including bathing, dressing, grooming, and transferring.
- Adhere to infection control policies and HIPPA guidelines to ensure a safe and healthy environment for patients.
- You will also assist with light housekeeping, meal preparation, and accompaniment to physician appointments or clinics.
Responsibilities and Requirements:
- Follows Plan of Care developed by RN/Therapist.
- Adheres to infection control policies and HIPPA guidelines.
- Assists with simple exercise programs, application of prosthesis and braces, ambulation, and transfers as outlined.
- Provides stimulation and encouragement towards independence and achieving the goals established by the professional staff and the patient's stated goal.
