Job Description:
- Deliver patient-centered care, prioritizing the patient's safety and comfort.
- Collaborate with the patient's physician and other healthcare professionals to ensure seamless care coordination.
- Develop strong relationships with patients and families through compassionate and responsive communication.
Requirements:
- A Delaware LPN license with one year of experience working in a healthcare setting is preferred.
- An associates degree or completion of an accredited LPN education program is required.
- Work experience in an acute or chronic care facility, or home health agency is preferred.
- Previous Home Healthcare and Long Term Care experience is preferred.
- You must be able to attend a two-week daytime orientation in our Camden office, followed by patient-specific field orientation on night shift.
- Tracheostomy and ventilator experience is a plus.
