Christopher Community, Inc. (CCINC) seeks a Resident Service Coordinator to join its growing organization! The physical work locations for this position will be at the Providence House Apartments located in Syracuse, NY.
CCINC is a not-for-profit Housing Development and Property Management company that assist low- and moderate-income families and seniors to secure suitable housing across Upstate New York. Christopher Community currently manages over 3,300 units of housing in more than 100 buildings, and administers the Rental Assistance Program in Onondaga County, which provides assistance to over 1,200 households. Since 1971, Christopher Community has maintained a proven track record in developing and operating affordable housing. We regularly receive very high ratings both from government and private entities for the way we manage our properties.

Requirements
The Resident Service Coordinator (SC) works in conjunction with the housing manager(s) and other management staff to facilitate services that empower residents to age in place and remain as independent and self-reliant as possible. They may provide limited or general case management; information and referral; plan educational programs, and coordinate volunteer opportunities. The SC reaches out to all residents on a regular basis to assess needs, promote involvement in activity that fosters wellness, educate residents on available services, and develop a network of contacts with service providers and agencies for resident referrals. The SC also monitors the provision of services to residents, providing documentation of their involvement with assisting residents, as well as community engagement efforts. This position will work at the Providence House Apartments, which is managed by Christopher Community, Inc.
- Reaches out to residents on a regular basis to note availability to help and educate them as to services that can assist with maintaining wellness and aging in place.
- Promotes wellness activities for all residents.
- Educates residents, families, and staff on available community resources.
- Assists residents in building informal support networks with others.
- Acts as a liaison between community agencies, service providers, and residents.
- Works as a team member with the facility manager and other housing management staff in serving residents/clients.
- Encourages residents to be proactive in meeting their social, psychological, and physical needs.
- Facilitates meeting of needs when necessary, but avoids creation of unhealthy dependence.
- Uses the least restrictive intervention necessary to alleviate a problem situation.
- May assist residents or coordinate training to understand lease and tenancy obligations.
- Does not perform any duties or functions that are duly assigned to management or are associated with management responsibilities.
Specific Assignments:
- Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory.
- Performs service management functions for all residents needing assistance.
- Provides general or limited case management for a resident when such service is not being provided by the larger service community.
- Educates residents on service availability, application procedures, residents' rights, etc., both individually and as a group.
- Reports all suspected abuse situations to the appropriate agency.
- Sets up volunteer support programs with service organizations in the community.
- Advocates and may negotiate on behalf of residents for adequate, timely, and cost-effective provision of services.
- Meets with service providers as needed and appropriate.
- Assembles a directory of community services and makes it available to residents, families, and management.
- Assists and works with management in identifying residents who need assistance.
- (Note) The SC does NOT provide continuous support services directly (direct service) or assist with other administrative work normally associated with the property’s operations. Additionally, their role is NOT one of an activity director.
Administrative:
- Documents contacts with residents, providers, and families.
- Maintains individual files on residents which contain required data, releases, and written documentation of all relevant services/events. Information is secured in locked files and passwords to ensure confidentiality.
- Maintains updated documentation on regular SC-related involvement in an accurate and timely manner according to recommended guidelines. Creates service management plan as appropriate.
- Pursues avenues for additional services that might be of benefit to building residents.
EDUCATION & EXPERIENCE:
- A Bachelor’s degree in Social Work, Gerontology, or other related field of human services is required.
- Master’s Degree preferred.
- Prior experience coordinating supportive services for seniors and/or persons who are disabled needed.
- Excellent oral and written communication skills needed. (An equivalent combination of education & experience may also be considered).
- Bilingual in Spanish preferred
