Responsibilities:
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Handle case assignments, draft service plans, review case progress and determine case closure
- Help clients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with medical team throughout the entire case management process
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record cases information, complete accurately all necessary forms and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and address motivational and psychosocial issues
- Adhere to professional standards as outlined by protocols, rules and regulations
- Documents all client encounters and contracts made on behalf of clients; completes and submits billing documentation as appropriate; maintains comprehensive client files, which may include documents held for safekeeping on behalf of the client.
- Identifies and provides emergency crisis services as necessary; makes immediate clinical assessments and responds according to accepted crisis intervention methods and techniques; coordinates other services as appropriate.
- Assesses patients, evaluates effectiveness of care plan and progress made by client, participates in patient treatment planning and case review with patient care providers.
- Maintains and reports applicable statistics regarding programs and client services.
- Performs miscellaneous job-related duties as assigned.
QUALIFICATIONS:
- Proven working experience in case management, including as a Nurse, Medical, Mental Health Care Manager or a related job
- Excellent knowledge of case management principles, healthcare management and reimbursement
- Demonstrate ability to maintain confidentiality for residents and staff
- Demonstrate organization, administration, and leadership skills
- Strong communication skills (oral and written), ability to communicate in nontechnical language with staff members, residents, and families
- Effective problem-solving, critical thinking skills, and conflict resolution skills
- Commitment to continuous improvement
- Read, write, speak, and understand the English language.
- Freedom from illegal use of drugs.
- Freedom from use and effects of use of drugs and alcohol in the workplace.
- Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in the position.
- Demonstrates effective time management skills.
- Must be 18 years of age.
- Able to communicate ideas well
- Able to question and listen to residents and coworkers for gaining understanding.
- Able to represent and promote activities and programs as a member of our team
- Able to organize and manage both information and time effectively
- Sufficient literary skills to read care plans and other procedures and to complete required reports.
Education and/or Experience: Bachelor's degree; at least 1 year of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
