Du Quoin Comprehensive Behavioral Health is hiring a full-time Integrated Care Coordinator.
The Integrated Care Coordinator will be responsible for coordinating patient care including scheduling, referrals, follow-up and all other aspects of patient care. This position will work closely with the Clinician and Treatment Team to facilitate access to and coordination of primary and acute physical health services and behavioral health care. Must be reliable and possess excellent skills in the following areas: written and verbal communication, organization and record keeping, proficiency in medication management, and extensive knowledge in chronic and acute illnesses, including behavioral health. A Social Services degree is required with social service and counseling experience.
