Job Description
The Patient Care Manager is responsible for the overall supervision and coordination of clinical services, ensuring high-quality patient care in the comfort of their own homes.
- Provides clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start of care visits.
The ideal candidate will have excellent leadership skills, a strong understanding of home health care, and the ability to work collaboratively with patients, families, and healthcare professionals.
Responsibilities
- Supervises and coordinates clinical services, ensuring compliance with laws, regulations, and agency policies.
- Evaluates patient needs, develops individualized care plans, and ensures timely implementation of services.

Requirements
- Current RN licensure in the state of practice.
