About the Role:
- You will be responsible for initiating, developing, implementing, and revising plans of care in collaboration with physicians and other healthcare professionals.
- This includes directly or indirectly supervising care provided by home health aides and licensed practical vocational nurses, providing instruction as needed, and assigning tasks according to State and federal regulations.
- Additionally, you will make initial and comprehensive nursing evaluation visits, ensure patients meet eligibility guidelines, accurately determine primary focus of care, and submit accurate documentation.
