In this role, you'll provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
- Care Coordination: Collaborate with other healthcare professionals to coordinate total care plans, ensuring continuity of patient care and achieving optimal outcomes.
- Nursing Evaluations: Conduct initial and comprehensive nursing evaluations, determining primary focus of care and developing plans within state-specific guidelines.
- Patient Assessments: Perform comprehensive patient assessments at designated times, developing appropriate care plans that meet individual needs.
- Eligibility and Necessity: Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
