Chronic Care Staffing provides Remote Physiologic Monitoring, commonly called Remote Patient Monitoring, or RPM, with our clinical care delivery team. Your patients will speak with a seasoned care coordinator, building a trusted relationship, increasing patient engagement and compliance. Our team will use state of the art monitoring software that captures patient data from wherever they use their devices. Our care team monitors patient RPM readings, devices and the overall health of your patient during every encounter.
