Your Role
- Provide monthly calls and support to patients enrolled in the CCM program.
- Utilize your nursing skills to educate patients and their families.
- Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
- MUST be bilingual (English/Spanish) - fully fluent in both
- MUST live in a CST or EST state to be considered.
- Active CMA Certification or Registration (nationally recognized with ID number)
- Experience in geriatrics or primary care
- Ability to sit and talk on the phone for long periods of time
- System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
- Must live and work full time in the US - you can work from anywhere within the US but not outside.
The Pay
- The CMA pay starts at $18.35 per hour (40hrs/week)
- The pay structure is a base pay plus a bonus per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
