Your Role
- Provide monthly calls and support to patients enrolled in the CCM program.
- Utilize your nursing skills to educate patients and their families.
- Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
- Active CMA Certification or Registration (nationally recognized)
- YOU MUST RESIDE IN NV, AZ, WA, OR, NM, or CO TO BE CONSIDERED
- Experience in geriatrics or primary care is required, in-office experience is a must for this role.
- Ability to sit and talk on the phone for long periods of time
- System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
- Must live and work full time in the US - you can work from anywhere within the US but not outside.
The Pay
- The CMA pay ranges from $18.35 - $20.27 per hour (40hrs/week)
- The pay is a base pay of $16.00/hr plus a bonus of $1.00 per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
