Overview
The Wound Care Nurse Practitioner (NP) works as a member of the medical team to provide wound care to patients with long-term care, transitional care, assisted living, and memory care homes. This position is responsible for providing wound Management for wounds collaboratively with the facility wound nurse/nursing team. The Wound Care NP is responsible for creating and maintaining a plan that results in successful wound care outcomes. The Wound Care NP will do on-site visits with patients and provide phone consultations and/or telehealth visits with nursing staff and providers with questions on the continuum of wound care.

The NP will work in skilled nursing facilities or patients’ homes and participate in weekly bedside wound rounds with the treatment nurse to evaluate and manage all acute and chronic wounds, surgical wounds, and dermatologic wounds, change dressings, perform medically necessary debridement, establish plan of care and work within facility protocols. NPs will conduct comprehensive patient assessments, paying special attention to factors that increase wound infection. Perform wound debridement and apply appropriate wound dressings. NPs will educate nursing staff, and maintain open communication with primary care physicians, families, and clinical teams on the product and interventions required for skin healing and maintenance.

Minimum Requirements

  • 2 to 3 years of wound care, Medicare Credentialed, and living within the El Paso Community.
  • Graduate of an accredited or professional nursing program required;
  • Graduate of a Wound Ostomy Continence Nurse Practitioner Program Preferred;
  • Knowledge of wound care-related products and procedures is required;
  • Knowledge of comprehensive wound management including wound assessment, formulation of treatment plan, recommendation of appropriate wound dressing, documentation, evaluation, education, and data collection;
  • Serve as a consultant in the management and treatment of patients with complicated fistulas, wounds, ostomies, and incontinence and provides recommendations for the effective and appropriate use of wound products and ostomy supplies. Educate patient with new ostomies and surgical incisions;
  • Work with the wound care nurse and facility leadership to determine opportunities for education of facility nurses with appropriate identification of wound types, prevention of wounds, and nursing care of wounds;
  • Provide timely and accurate documentation of patient plan of treatment within 24 hours of patient visit;
  • Orders, reviews, interprets, and integrates diagnostic studies and laboratory findings for initiating or modifying patient treatment plan(s);
  • Knowledge of up-to-date topical wound care products and therapies, including ordering, ap[application, and management of products on wounds and ostomies, also including negative pressure wound therapy;
  • Meeting the established WHC Standards of Nursing Practices in the management of all assigned patients;
  • Collaborate with multidisciplinary teams to perform bedside skin care procedures like wound debridement, skin graft cleaning, and dressing change.
  • Review medical histories, and symptoms with the development of a comprehensive treatment plan;
  • Collaborate with the patient’s primary care provider when comorbidities are contributing to our affecting the healing of wound to determine a comprehensive plan;
  • Update the primary care provider on the status and management plan of the wound as appropriate;
  • Prescribe medication and/or durable medical equipment to assist in healing wounds;
  • Strong organizational and analytical skills;
  • Responsible for collaborating and rounding with wound care nursing staff to assess the treat patients’ wounds;
  • Review patient' prior medical records and maintain their current medical record related to the wound diagnosis and contributing comorbidities;
  • Assist with the identification of wound care quality measures, and provide data and documentation for tracking of quality measures;
  • Form and maintain relationships with facility staff, patients, and family members;
  • Team player, able to work independently

Qualifications:

  • Certified Wound Care Nurse (CWCN) Certification, CWS or WCC
  • Graduate of an accredited or professional nursing program required; MSN or DNP preferred
  • Bilingual in English and Spanish
  • Clean driving record and valid drivers license
