Your Mission (Should You Choose to Accept): As our next Community Case Manager, you’ll be the hero of hope for our clients. Every day, you’ll use your knowledge, empathy, and organizational support to help people in need access the resources they require to succeed.
What You’ll Do:
- Be the first point of contact and build trust with clients facing complex life situations.
- Develop individualized service plans to guide clients toward achieving their goals.
- Connect clients with community resources, services, and support networks.
- Advocate on behalf of clients for housing, healthcare, financial assistance, and more.
- Track and document progress, celebrating victories big and small!
- Collaborate with a passionate team of colleagues and partners to create lasting impact.
About You:
- You’re a compassionate and patient listener who believes in people’s potential.
- You have a knack for juggling multiple tasks without dropping any balls.
- You’re a problem-solver at heart who thrives in fast-paced, dynamic environments.
- You’re organized, detail-oriented, and love seeing projects through to completion.
- Previous experience as a Case Manager or in social services is a huge plus!
- Bachelor’s degree in Social Work, Psychology, or a related field preferred (but if you’re a life-learned expert, we’d still love to hear from you).
