Key Responsibilities :
- Case Management :
o Coordinate admissions, assessments, and service plans for clients residing in the shelter.
o Maintain accurate documentation of all client-related contacts and ensure compliance with medical, dental, and contract requirements.
o Serve as a liaison for clients, families, referral sources, and social service agencies.
o Provide ongoing case management to address psychosocial, behavioral, and medical needs.
o Monitor client progress and collaborate with a multidisciplinary team for permanency and discharge planning.
o Utilize community resources to support clients’ needs and create effective discharge plans.
o Transport clients as needed for appointments and court visits.
o Conduct in-service training for staff to enhance their skills and understanding of shelter services.
o Train direct care staff in implementing individualized service plans.
o Perform ongoing chart audits to ensure quality assurance and compliance with policies.
o Recognize and intervene therapeutically in crisis situations.
o Represent Chrysalis Health with professionalism in all communications, even in high-stress situations.
o Consistently uphold the company’s mission, vision, and goals in all activities.
Qualifications / Requirements :
Work requires Group Home Case Manager to be a road warrior, have a flexible schedule and ability to work beyond agency hours of operation to ensure that persons served needs are met; this may include evening hours and weekends.
Bachelor’s degree from an accredited university or college (any major), AND 3 years of experience working with children with serious emotional disturbances.
Bachelor’s degree from an accredited university or college in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling, and related field with one year of experience working with children with serious emotional disturbances
o Familiarity with DSM-IV and DSM-5.
o Experience in mental health services and crisis intervention is highly desirable.
o Proficiency in clinical documentation, HRIS management systems, and computer applications.
o Strong understanding of compliance requirements and chart maintenance.
o Valid driver’s license, proof of insurance with minimum liability coverage of $10,000 / $20,000 / $10,000, and a safe driving record (no more than six points on MVR).
o Must complete annual criminal background checks and training requirements.
o Bilingual candidates are highly encouraged to apply.
