Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals through various challenges, including addiction and mental health issues. This position requires strong organizational and interpersonal skills, a commitment to advocacy, and the ability to navigate complex systems to ensure clients receive the necessary resources and support.
Responsibilities
- Conduct comprehensive discharge planning with clients to identify their needs and existing supports to encourage their continued sobriety after discharge.
- Provide crisis intervention and management to clients in distress.
- Facilitate access to resources such as medical, dental, vision, education, and workforce development programs.
- Mentor clients through their recovery process while promoting self-sufficiency.
- Supervise casework activities and ensure compliance with relevant regulations and policies.
- Collaborate with multidisciplinary teams including social workers, healthcare providers, and community organizations.
- Maintain accurate documentation of client progress and case notes in accordance with organizational standards.
- Advocate for clients' rights and needs within the community and service systems.
Qualifications
- Bachelor’s degree in Social Work, Psychology, or a related field is required.
- Experience in adolescent addiction counseling, or adolescent behavioral health is highly desirable.
- Strong knowledge of Medicaid processes and child welfare systems is a plus.
- Proven skills in crisis management and intervention techniques.
- Excellent communication, mentoring, and interpersonal skills to effectively engage with adolescent populations.
- Commitment to maintaining confidentiality and ethical standards in all interactions.
