Position: Transitional Housing Coordinator
Summary:
The Transitional Housing Coordinator will coordinate Chrysalis’ Transitional Housing Program, ensuring that all guidelines and polices are followed and that all reporting requirements are met. This position will promote a program culture of respect and maintain a high degree of confidentiality.
Duties and Responsibilities:
- Maintain resident files in accordance with licensure
- Attend all scheduled group staffing/supervision meetings
- Attend individual supervision with transition team as needed
- Develop case management plans utilizing measurable objectives, update as needed and/or every six months.
- Document resident’s progress towards goals in resident progress notes weekly
- Provide referrals to other community agencies as needed
- Work with transition team members as needed on statistics compilations
- Follow and maintain all ethical and professional guidelines as set forth in Chrysalis’ policies and procedures
- Be willing to set up apartments when moving in residents to provide a welcoming environment
- Attend training as outlined in Chrysalis Training Plan
- Participate in application reviews interviews and completing intakes of new residents
- Offering case management and living skills groups to East and West sites
- Provide crisis case management during non-business hours on a needed basis
- Provide case management functions to all residents
- Must be willing to drive to all Transition sites and apartments weekly
- Maintain good working relationship with property managers, and constant open team communication
- All other duties as assigned
Knowledge, Ability, Experience Requirements:
- Bachelor’s degree in human services or related field and/or a combination of education and experience
- Experience and/or knowledge of domestic violence
- Ability to perform crisis intervention
- Ability to perform case management functions
- Ability to maintain working relationships with supervisor, associates, clients, and community agencies
- Ability to communicate well both verbally and in writing
Other Requirements:
Must provide own transportation and automobile insurance and have the ability to travel throughout the community; must be able to pass a fingerprint and background clearance as required by Chrysalis and ADHS licensure requirements; must maintain current CPR and first aid training as required by Chrysalis policies; Routinely moves equipment weighing up to 25 lbs and may be required to ascend/descend stairs and remain in a stationary position during events, and occasionally will need to be available evenings and weekends.

Pay: $20.00 - $23.00 per hour
