The Care Coordinator RN is responsible for coordinating access to complex medical, dental, mental/behavioral services for each child enrolled in Care4Kids. Responsible for the development of the comprehensive coordinated services plan and ongoing monitoring of the plan for each enrolled child. Integrates and oversees all of the services provided to the foster care child and family. This position will utilize problem solving techniques and collaborate with other staff members and stakeholders to make referrals to appropriate community resources.

Hours: Full Time

Location: Remote but must be commuting distance to Milwaukee

What you will need:

• Registered Nurse (BSN preferred) with a current license to practice professional nursing in the State of WI

• Minimum of two years of nursing practice experience preferred and knowledge of case management theory.

• Previous training and experience working with children with special health care needs or in foster care preferred.

• Has a thorough understanding of the contract requirements related to the role of the Health Care Coordinator and the outcome measures for Care4Kids.

• Excellent communication skills written and oral.

• Strong critical thinking, problem solving and attentiveness to detail required.

• Demonstrated experience using Microsoft Office, Internet/Intranet, clinical and other information systems and technology

• Independent judgment, analytical ability, systems thinking and interpersonal communications to work in complex systems.

• Requires a valid driver's license, acceptable driving record, minimum liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.

• Sensitivity to cultural diversity and varying coping styles of families under stress. Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds strongly preferred.
