- assess and plan healthcare that is individualized to the patient
- review and interpret medical, surgical, and social histories
- perform screening procedures for patients
- interpret laboratory and other diagnostic data and recommend treatments or actions

The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. POSITION DESCRIPTION :
The Nurse Practitioner or Physician Assistant (APP) is responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.
The APP conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients goals and needs of care.
The APP is supported by a Clinical Manager / Director, as well as a Medical Director.
Specific responsibilities include, but are not limited to :
The company uses a mobile-based EHR and provides full training in their documentation system.
The APP will work in one (1) SNF location, seeing the same patients and working with the same support team on a daily basis.
