Essential Duties and Responsibilities
Include the following; other duties may be assigned:
- Provides direct patient care as defined in the LPN State Nurse Practice Act
- Implements current nursing practice, following a patient assessment and an approved Plan of Care initiated by the Registered Nurse.
- Collaborates with the RN to review/revise the plan of care, including a discharge plan, based up on the assessment of patient needs.
- Provide accurate and timely documentation of patient services to reflect the Plan of Care.
- Assess and provide patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
- Participates in coordination of agency services, appropriately reporting the identified needs for agency Interdisciplinary Group members.
- Provides appropriate pain/symptom management; evaluates patient’s response(s) to treatments/medications.
- Participates in personal and professional growth and development. Also participates in the organization’s quality management program.
- May be in multiple locations in a work day, including patient homes and CHSGa Home Health offices.
- Ability to establish and maintain effective working relationships with associates, department heads, administrators, member organization leadership teams, and the general public.
Minimum Qualifications
- Nursing degree from an accredited college or university or is a graduate of an approved LPN program.
- Current license / registration by state of hire as a LPN.
- Current CPR certification.
- Preferred experience in medical/surgical nursing.
