Primary Responsibilities
- Provide personalized care services, including bathing, dressing, grooming, and toileting, promoting dignity and respect.
- Assist residents with daily activities, such as meal preparation, feeding, and medication management, while encouraging independence and self-sufficiency.
- Monitor residents' health status, reporting any changes or concerns to nursing staff and maintaining accurate records.
- Collaborate with interdisciplinary teams, including nurses, therapists, and social workers, to coordinate care and ensure seamless transitions.
- Develop and implement individualized care plans, tailoring services to meet the unique needs and preferences of each resident.
Preferred Qualifications
- High school diploma or equivalent required; CHHA certification minimum.
- STNA/CNA certification preferred; CPR certification also desirable.
- Previous experience in long-term care, assisted living, or home healthcare highly valued.
- Strong communication, interpersonal, and organizational skills essential.
- Ability to work independently and as part of a team, prioritizing resident needs and responding to emergencies.
