Care Medical Practice is looking for a compassionate and skilled Nurse Practitioner or Physician Assistant to perform in-home assessments and assist with chronic disease management. In this role, you'll work closely with a multi-disciplinary team to deliver high-quality, personalized care. The ideal candidate is experienced in primary or home care, committed to improving patient outcomes, and eager to make a meaningful impact in underserved communities.

Compensation: From $118,000 to $125,000 per year

Position Overview
Care Medical Practice is looking for a compassionate and skilled Nurse Practitioner or Physician Assistant to perform in-home assessments and assist with chronic disease management. In this role, you'll work closely with a multi-disciplinary team to deliver high-quality, personalized care. The ideal candidate is experienced in primary or home care, committed to improving patient outcomes, and eager to make a meaningful impact in underserved communities.
Key Responsibilities
- Perform comprehensive in-home assessments and assist the primary care team in managing chronic diseases.
- Conduct physical examinations, medication reviews, and lab draws; provide wound care and mental health screenings.
- Collaborate with the primary care provider to identify gaps in care and provide recommendations.
- Ensure accuracy in risk capture, maintain high-quality metrics, and contribute to patient satisfaction by improving health outcomes.
- Educate patients on disease processes, medication adherence, and preventative care.
- Travel 5 days a week to provide care to patients in their homes.
- Current state licensure as a Nurse Practitioner (FNP, APRN) or Physician Assistant.
- Nurse Practitioner certification from ANCC or AANP.
- 2 years of clinical experience in primary care, home care, or similar settings.
- Knowledge of chronic care management and medication options.
- Current DEA licensure and BLS/ACLS certification.
- Experience in family medicine, internal medicine, gerontology, or palliative care.
- Strong communication and relationship-building skills.
