What are the responsibilities and job description for the Care Coordinator position at Circle of Care St. Louis?
The Care Coordinator is part of the Social Service Division at Circle of Care St. Louis. In this role, the Care Coordinator will provide the following services:
- Conduct intake interviews and needs assessments for new clients
- Research and maintain a directory of community resources, including food banks, housing assistance, job training programs, etc.
- Help clients fill out applications and paperwork for government programs
- Maintain detailed records and case files tracking client interactions and progress
- Participate in training to learn about available resources and social services procedures
Qualifications
- Associate Degree in Social Work or related field
- Strong communication, listening, and interviewing skills
- Ability to collaborate with clients compassionately and non-judgmentally
- Proficient in Microsoft Office Suite and database management
- Interest in social services and/or nonprofit
- Must have reliable transportation as this position travels
This position is 30 hours per week, Monday through Friday, 11:00 a.m. to 5:00 p.m., and is an excellent opportunity for students interested in social work, case management, and direct client services.
