Summary of position:
The Health Center Case Manager is responsible for coordinating comprehensive care to address the medical, social, and emotional needs of patients. Serving diverse populations, which includes individuals experiencing homelessness, chronic illnesses, mental health issues and substance use challenge, the Case Manager must support solutions, anticipate challenges and take initiative in overcoming obstacles to care. The case manager will collaborate with the Behavioral Health Consultant (BHC) to support attainment of care plan goals created during the BHC consultation. The case manager provides patient care in a manner consistent with the values of Circle the City.
Essential Duties:
Duties include, but are not limited to:
- Provides proactive support to patients working towards attaining BHC care plan goals including connection to mental health and substance use services, housing resources, medical care and other unmet social determinants of health in a collaborative patient centered manner.
- Collaborates with BHCs on patient care plan progress and provides feedback regarding updating the plan
- Proactively and efficiently manage assigned case load, reporting updates at clinic meetings
- Build and maintain a positive working relationship with health center staff to ensure successful patient outcomes.
- Coordinates and fosters collaborative relationships with community resource partners
- Serves as an advocate for patients to reduce barriers to care and services.
- Completes documentation within agency timeframes in the EMR system.
- Adheres to Health Insurance Portability and Accountability Act (HIPAA) guidelines
Qualifications
Qualifications:
- Bachelor’s degree in social work, psychology, public health, or a related field preferred.
- High School Diploma or equivalent required.
- Minimum of 2 years’ experience in case manager in social services or health center required.
- Spanish speaking preferred.
- Strong interpersonal skills with an ability to work effectively with others.
- Experience in working with culturally diverse individuals who struggle with homelessness preferred.
- Demonstrates excellent oral and written communication skills.
- Must demonstrate critical thinking, problem-solving, organizational and time management skills
- Possess and maintain a valid Arizona driver’s license and has use of privately owned vehicle
- Requires a current Arizona Department of Safety Clearance Card.
- Basic Life Support (BLS) Certification
Physical and Mental Requirements
- Position requires extended periods of sitting and standing.
- Position requires regular bending and reaching, including transfer of patients.
